L&I Claims

L&I Claims that are state funded: If you are opening a new claim or re-opening an old one, all payments will be made by L&I, and you will not be billed.

L&I Claims that are employer funded; i.e. “Self Insured”: If you seek to either open or re-open a claim involving an employer who uses L&I’s “Self-insured” option, then you must be represented by a labor law attorney before making an appointment with Work Injury Medicine. You will not be billed.

Self-pay

I accept payment directly from patients in advance for non-L&I consultations, including non-L&I electrodiagnostic evaluations. Rates charged are identical to those by L&I.

Other Insurance

At this time, I do not accept other insurance except as noted above.

DOT Medical Exams

DOT Exams are conducted at ARCpoint Labs. Payment for DOT Exams is arranged between you and ARCpoint Labs. Schedule your appointment with ARCpoint Labs 360 922-0275.
